Newly released video shows final moments of UPS flight 2976

LOUISVILLE, Ky. (WAVE) - New video has been released of the final moments of UPS flight 2976. The video was recorded by multiple airport security cameras, including one that captured just how low to the ground the plane was.

The National Transportation Safety Board has issued its preliminary report into the crash of UPS 2976 , an MD-11F bound for Honolulu that crashed on takeoff at Muhammad Ali International Airport.

The preliminary report was released 16 days after the crash.

The three crewmembers were killed, as were 11 people on the ground. There were also 23 injuries as a result of the crash.

The initial report shows that the taxi and takeoff roll were uneventful. The Flight Data Recorder contained about 63 hours of data that spanned 24 flights, including the crash. Initial investigation revealed that the left engine and left pylon detached during takeoff.

The airplane cleared the blast fence beyond the end of runway 17R, but the left main landing gear impacted the roof of a UPS Supply Chain Solutions warehouse at the southern edge of the airport before crashing into the supply yard on the other side of the fence.

The wreck was incredibly broken up but the NTSB collected the pieces and cleaned them. The cleaning of the fracture surfaces, examination of the left pylon aft mount lug fractures found evidence of fatigue cracks in addition to areas of overstress failure.

On the aft lug on both the inboard and outboard fracture surfaces, a fatigue crack was observed where the aft lug bore met the aft lug forward face. For the forward lug’s inboard fracture surface, fatigue cracks were observed along the lug bore, which would have been holding the parts in place.

Fatigue cracks are microscopic breaks that can develop overtime in aircraft due to pressurization, takeoff and landing.

The report compares the crash of UPS 2976 to the 1979 crash of American Airlines flight 191 in Chicago. During the takeoff rotation of AA 191, the left engine and pylon assembly and about 3 ft of the leading edge of the left wing separated from the airplane and fell to the runway. The DC-10 rolled on its side in a similar fashion before crashing into a field.
